Linking Leeds is the new city-wide Social Prescribing service for people in Leeds, which works in partnership with primary care to help people access relevant services and support groups within the community.
Live Well Leeds is a new community mental health support service run by Touchstone, Community Links and Leeds Mind. The service supports those with mild to moderate mental health needs within the Leeds area.

This service is run in partnership with Carers Leeds and provides a universal wellbeing support service to people in Leeds living with younger-onset dementia (under the age of 65), their families and carers.
