PDS is part of the Community Links Specialist Trauma Informed Resettlement Services (STIRS).
The PDS is a service across Yorkshire and The Humber that offers case review and signposting to local clinical commissioning groups and specialist commissioners for clients that are detained in (or are at risk of escalating into) secure hospital beds, or where pathways back to the community are blocked leaving clients ‘stuck’.
The overarching aim of the Pathway Development Service (PDS) is to support systems striving to deliver trauma-informed, integrated care for people who experience the world as profoundly unsafe (often associated with the diagnostic label of a ‘personality disorder’).
The team works in collaboration with the wider PDS multi-disciplinary team, primarily with clients with high risks and complex needs, many of whom have attracted diagnoses of personality disorder.
Housing and Resettlement Caseworkers offer housing reviews to any clients where housing routes to the community are challenging. They negotiate and consult with suitable supported housing placements and work closely with providers to upskill their teams to support clients in a trauma-informed manner.
